These steps will assist you in using Medicare to gain your immunisation history.

STEP 1: SIGN IN

1. Go to MyGov and open your Medicare Linked service

2. Select the History and Statements from the top Menu Bar

3. Select Immunisation History Statement from the drop-down menu

STEP 2: SELECT THE PERSON

1. Select the person's name whose statement you wish to view in the drop-down box

2. Select View History Statement to continue

3. This will automatically download your immunisation History Statement on to your computer

4. Open your Immunisation History Statement, then either Print or Save.
